Medical Concerns

Please call the School Nurse's Office at 767-5410 to discuss  any of your child's health concerns or any injury requiring a gym modification.  
 
If your child has any other medical issue requiring medication during school hours,  you may  access the medication form from the Medical Office website.
 
If your child has a food allergy requiring an Epi-pen and Benadryl, you and your doctor must complete the Food Allergy Action Plan.
 
*Epi-pens will accompany your child on all field trips*
 
If your child has Asthma, requiring an inhaler or nebulizer at school, a medication form must be completed by you and your doctor.
