Medication Requirements

New York State law prohibits the taking of any medication by students during school hours unless the school nurse has been furnished with the following information:
 
 1.  Written request from parents
      2.   Doctor’s note stating:
-         Name of Prescription
-         Dosage and time to be given, and number of days
-         Reason for taking medication
       3.  Medication must be properly labeled in a bottle by the pharmacy.
 
These requirements apply to Tylenol, Advil, cough medicine and any other over-the-counter medicine, as well as prescription medication.
 
All medication will be stored in the Health Office and dispensed only by the School Nurse according to written instructions only.
 
A medication authorization form may be obtained from the Nurse’s Office and must be renewed at the beginning of each year or when medication is changed.
